Job Description: * Conduct home and hospital visits for potential clients, where about 70% of time will be spent doing these visits. * Assess and submit Household Means Testing application for clients. * Raise referrals for eligible clients via IRMS to required services. * Work with referral sources for client’s discharge care plan. * Conduct timely assessments to identify client’s care needs. * Work with nurse manager, centre manager and home care manager to review client’s care plan and interventions. * Assess and assist client to apply for financial assistance. * Any adhoc tasks given by CEO/Home Care manager.
Requirements: * Diploma or Degree in any fields are welcome. * Candidates with Nursing, Medical Social Work or Care Coordinator backgrounds are a plus and welcome to apply. * Experience in community care or hospitals would be advantageous. * Able to work independently with little supervision. * Proficient in Microsoft Office and comfortable with IT. * Able to speak local languages and dialect (preferable).
